Sustaining High Levels of Motivation

I personally experience rather fluctuating levels of enthusiasm/motivation. I can maintain extremely high levels of motivation for about one day out of the week, sometimes two. By high levels of motivation, I mean every second of the day I'm moving/thinking as fast as I physically can, trying to be as efficient and effective at possible. However, it's always temporary. Eventually I'll get discouraged, my enthusiasm/energy levels will drop back to average until something else jump starts my motivation back to overdrive. Fortunately, I've never even considered giving up. I just know I would be a hell of a lot more effective if I could sustain that level of enthusiasm for more days of the week. Part of it probably has to do with bad inner game (beliefs that need re-framing/reinforcing), but none-the-less it's not helpful.
So, my question is, is there anything you guys do to proactively jump start your motivation when it subsides?

Good nutrition, meditation and working out do the trick for me.
When I don't do those I'm 1/3 as effective as usual.

When you're a young guy there's going to be highs and lows. That's life. So first and foremost realize that.. then you won't get all despondent and resist the low points, and so they won't overly persist.
The end result you're looking for, is to anchor in yourself the cause (you taking action) with the result.. success, or improvement, or a desired result. This builds in you and gives you certainty in yourself, and whatever you put your mind and body to do. As guys we also have the added boost of getting a boost out of just taking simple action.
You can also work on completely taking emotion and ego out of the game by being in an active meditative state, and acting simply because it is the right thing to do. "Right Action".
In your case as well, one of the most basic fundamentals you're lacking is a physical exercise regime. Do some sprints outside and some push-ups and pull-ups and you will seriously boost your testosterone and natural drives.

Break your goals down into smaller, shorter-term goals. Then reward yourself after achieving these goals. Like seriously break it down into the smallest mini-goals it can be -- on a daily, shit even hourly level. It becomes overwhelming when you're looking at the big picture, and is one of the common reasons motivation will waiver.

This is the main premise behind the life management training I do with my guys.
